City Registration Form - bring a check payable to the City of Sunrise for $80 (city residents) or $150 (for non city residents). Or, fill out the credit card information. Please note that your check will be deposited or your credit card will be charged ONLY if you end up on the team. You will not be charged for trying out, but we need all of the paperwork filled out ahead of time.
